Weight Gain by Trimester

For a normal-weight woman carrying twins (target: 37–54 lbs total), weight gain is not evenly distributed across pregnancy:

First Trimester (weeks 1–13)

1–4.5 lbs total

Minimal gain; focus on folic acid and avoiding nausea triggers

Second Trimester (weeks 14–27)

~1–1.5 lbs/week

Fastest growth period for babies; increase calorie and protein intake

Third Trimester (weeks 28–38+)

~1 lb/week

Slowing pace; may plateau near term; monitor for edema

Average delivery for dichorionic twins is ~38 weeks; monochorionic twins ~36–37 weeks. Weight gain patterns should account for this shorter timeline compared to singleton pregnancies.

Nutrition for Twin Pregnancy

Nutrition requirements are substantially higher for twin pregnancies. Focus on nutrient density, not just calorie quantity:

Protein: 150–175g/day

Supports growth of two babies. Sources: Greek yogurt, eggs, chicken, legumes, cottage cheese.

Folate: 1mg/day (1,000mcg)

Higher dose than singleton pregnancy. Prevents neural tube defects for both babies.

Iron: 30–60mg/day

Twin pregnancies demand more iron. Monitor for anemia — fatigue and shortness of breath are warning signs.

Calcium: 1,000–1,300mg/day

Especially critical as both babies mineralize their skeletons simultaneously.

DHA: 400–600mg/day

When Weight Gain Is Concerning

Excessive gain is associated with gestational diabetes, hypertension, preeclampsia, and delivery complications. Stay within guidelines and discuss with your OB. Watch for these warning signs:

Rapid sudden weight gain (5+ lbs in a week)

May signal preeclampsia or HELLP syndrome — fluid retention, not fat gain. Contact your provider immediately.

Inadequate gain before 28 weeks

Insufficient nutrition during key growth phases is linked to low birth weight and preterm birth.
