Twin Pregnancy Weight Gain Guide: How Much Is Healthy?
Everything you need to know about healthy weight gain during a twin pregnancy — IOM guidelines, trimester targets, and nutrition strategies.

IOM Weight Gain Guidelines for Twins
The Institute of Medicine (IOM) 2009 guidelines provide the most widely used recommendations for twin pregnancy weight gain, based on pre-pregnancy BMI. Two babies, two placentas, more amniotic fluid, increased blood volume, and more uterine and breast tissue all contribute to higher total weight gain than a singleton pregnancy.
| Pre-Pregnancy BMI | Category | Recommended Gain |
|---|---|---|
| Below 18.5 | Underweight | 50–62 lbs (23–28 kg) |
| 18.5–24.9 | Normal weight | 37–54 lbs (17–25 kg) |
| 25.0–29.9 | Overweight | 31–50 lbs (14–23 kg) |
| 30.0+ | Obese | 25–42 lbs (11–19 kg) |

Weight Gain by Trimester
For a normal-weight woman carrying twins (target: 37–54 lbs total), weight gain is not evenly distributed across pregnancy:
First Trimester (weeks 1–13)
1–4.5 lbs totalMinimal gain; focus on folic acid and avoiding nausea triggers
Second Trimester (weeks 14–27)
~1–1.5 lbs/weekFastest growth period for babies; increase calorie and protein intake
Third Trimester (weeks 28–38+)
~1 lb/weekSlowing pace; may plateau near term; monitor for edema
Average delivery for dichorionic twins is ~38 weeks; monochorionic twins ~36–37 weeks. Weight gain patterns should account for this shorter timeline compared to singleton pregnancies.
Nutrition for Twin Pregnancy
Nutrition requirements are substantially higher for twin pregnancies. Focus on nutrient density, not just calorie quantity:
Protein: 150–175g/day
Supports growth of two babies. Sources: Greek yogurt, eggs, chicken, legumes, cottage cheese.
Folate: 1mg/day (1,000mcg)
Higher dose than singleton pregnancy. Prevents neural tube defects for both babies.
Iron: 30–60mg/day
Twin pregnancies demand more iron. Monitor for anemia — fatigue and shortness of breath are warning signs.
Calcium: 1,000–1,300mg/day
Especially critical as both babies mineralize their skeletons simultaneously.
DHA: 400–600mg/day
Omega-3 for brain and eye development. Fatty fish 2–3×/week or high-quality supplements.
Calories: +600/day
Over pre-pregnancy maintenance from second trimester onward.
When Weight Gain Is Concerning
Excessive gain is associated with gestational diabetes, hypertension, preeclampsia, and delivery complications. Stay within guidelines and discuss with your OB. Watch for these warning signs:
Rapid sudden weight gain (5+ lbs in a week)
May signal preeclampsia or HELLP syndrome — fluid retention, not fat gain. Contact your provider immediately.
Inadequate gain before 28 weeks
Insufficient nutrition during key growth phases is linked to low birth weight and preterm birth.
Significant weight loss in 2nd/3rd trimester
Hyperemesis gravidarum or other complications — requires medical management.
